(A comprehensive medical, dental, vision, and prescription coverage plan)

Who Is Eligible? Anyone Meeting the Following Requirements:

  • California Resident(s)
  • Be between the ages of 1 to 19
  • Be a U.S. citizen or have legal residence in the United States
  • Annual household income that is in accordance with the rules (please call for more information)

    • If you are eligible, based on the above list and would like more information or to register, please call one of the offices of LaSalle and we will assist you.

    • Documentation Needed to Apply for Healthy Families:

  • Birth certificate or proof of legal residence in the country
  • Social Security number of the applicant
  • Proof of income (copy of monthly checks, or copy of tax filings for the previous year)
  • Identification of the parent(s)
